Applicants need to provide the following information in their application:

• Three years of loss runs

• Payroll audit

• Indemnity agreement

• Payroll questionnaire

All prospective members must be in a healthcare-related field that qualifies under the bylaws of The Group. They also must be members of the New Mexico Health Care Association and located in New Mexico.
